Who reports to the bureau as "ncb/perfor"? It is showing as a revolving account which I am assuming is a credit card, possibly in the state of Ohio.

If you are looking at a commercial report that someone gave you, then it might not show the address/phone. In that case you can call the credit bureau(s) and ask them.
